1. Duly licensed to practice medicine in the Philippines.
2. Member in good standing of the Philippine Medical Association
3. Must have completed satisfactorily a post-graduate course in Occupational Health and/or Industrial Hygiene given by the recognized institutions.

Other requirements:

1. Xerox copy of Certificate of Post-Graduate Course in Occupational Health and Safety
2. 2” x 2” ID photo
3. PMA and PRC I.D. card (Xerox copy)
4. Payment of P750.00 (Entrance fee = P150.00; Annual Dues = P300.00; Mutual Aid = P150.00; Local Dues = P150.00) to be made upon approval of National Officers and Board Members.
